This is a balanced build with solid offence and defense. It utilizes Demi and high paragon lvl bonuses. For monk demi and high level extra ability points means probably the most of any class as they gain 2 points of ac for +2 increase on all stats (dex and wis), so +4 demi's +2 PL and +2 from 16 items you net 8 points of ac vs lvl 60 with +14 items.
Pro's

Wm crits (bastard sword 14-20/x5)
Full weapon spec for +20
Monk defense benefits such as:
-71% conceal
-crit immunity
-total mind immunity
-other monk freebies like imp evasion, free survivebility feat, monk speed
You only self buff shadowshield and Wrap + toggle conceal on, it lasts 20mins. (If party stops for afk then toggle off)

Edit: used CA pandect, sneak attack or disarm are good options. High lvl boost to ab helps vs normal versions
Asmo gloves gives 5d6 sneak, wrap is 3d6 if use some other arty. Generaly I dont give sneak too much value since you hit big targets that are immune or quickly dismantle crittables where sneak doesnt play big part anyways. Also you want to be mobs target, so you dont get sneak anyways, secondary tanks then can sneak them, like rogues.

Discussion/Notes
When I say you only buff shadowshield I mean you should also always carry CL 45 FoM item with you incase your casters are slackers, when playing with friends they should be auto-fom'ing everyone. Wrap of dark prince is optional buff that comes with acquiring the said item. 20% damage increase makes it really nice to have and definitely something worth to burn your gold/items to trade for. I suggest carrying 2 per tank so occasional deaths or random afk moments wont ruin your dmg. 3x5mins per cloak. Other gear to really look for:
Strongheart or highly upped bur with 18ac and phys imm or resist (If go phys imm, then use boots for resist)
Robes of dim mak's for epic will/ele resist
Graz'zt's abyssal helm and use "of the salamander" -aug for ESF listen (I'm phys imm addict, take it everywhere you can)
I use asmo gloves (those imms rock) but for example belt is decent option if you have some great rand phys bur bracers
Leg boots of defence (exo resist, +16soak)
Belt anything that adds something nice. I use limbo 30/- phys resists, but for example azz 1 belt for 5% phys imms is an option.
Breach rings of multiple kind.

Well rounded Universal Contumacy is my favorite imm ring, other bur rings with single type exo is capped at 35%, good UC has 50%+ exo and 150% ele (ele is lower than well rand regular bur with 2x90+% and 3x 10+%) but I tr yto gear for exo, cus ele gets buffed with shunt most of the runs.
Zenythri works as an alternative subbie:
Str 14 > 18 + 20 lvl +4 feat +4 demi +2 PL +16 gear = 64

Halfling vs Human. I like the free ab/ac, having relatively low saves the halfling luck, which enables leg luck is quite decent vs free of choice human lvl 1 feat, which I'd most likely end up spending on it anyway. Extra skill point would negate penalties on skills, 5xD's dont suffer from this much apart from the very deepest of runs. Matter of preference, also take into account your weapon selection, halfling: bastardsword + blud for pwa: Human scythe + blud, no real power difference which ever end up going. Halfling doesnt get stuck as easily, which is nice with monk speed and also looks better, hence that's my choice.
There's alternative dimension where one drops str feats and picks up more saves to be less reliant on gear, but in this one the 1's happen too often so I choose to wear the items. Had I not have the items, dropping 2x str for great fort and leg fort to bump fort to 80 would be considerable option. In that same dimension one could also drop starting dex by 2 and up con by 4 and swap greater wep foc for toughness. Pwk checks current hp not full hp however. To further up the hp that alterdimentional monk would take paragon toughness instead of enviromental adaptivity. Total difference from 1200~ hp to 1700'ish at cost of ab/ac and higher pens. The hp swap isnt bad choice for any lazy player who doesnt want to heal as often. Tho once you stop being immune to damage when hit, the pens accumulate fast on the incoming damage. That extra hp could mean you die on same amount or even less amount of hits than a fellow party member with higher imms.
To further derail on the hp matter, another option is to lower str to get hp w/o touching the adaptivity feats. As long as ab stays over 105 I'd consider it a safe zone to adjust the build. Not to mention those lesser pens also means less ab loss, notably losing ab when being under effect of GR.
Can I post non demi version? With monk conceal ac around 130 should do decently in hells, I dont see a real reason why it wouldn't be viable, just not nearly as sturdy as the lvl 80 big bro is. In other words, you wont be a real tank, just an off-tank with 2hand pewpew. I wouldn't advice on going deep abyss before few hel cycles and lvl around 70. AC in abyss is higher and enemies punish you more for getting hit, so demi bonuses highly recommend.
Can I post open/secret/Ur version? Sorry, too lazy to do that. Monk's shine with extra stats as they scale with str/dex/wis and pretty much every endgame tank need 14-16int minimum for skills. For mid game playing some other monk will probably suit you better.
