I have a 5xD rogue I need to reincarnate (for The Abyss). He has a lot of "undroppables" like 5-6 Hell Store Rods all with maximum charges, an Electrifier (99 charges currently) and a Moliated Guildmaster's Glint. Will all of these items be returned unmodified when I reincarnate?
Also, he is attuned to the PoP and his skull says he can go to Nessus. I know I lose the skull when I reincarnate, but do I have to get a new one from Avernus BEFORE I re-attune to the PoP? I guess it must be so, but it seems odd.

The rods will be returned with the current number of charges. The electrifier will be returned empty. The Glint will be returned. You will receive a blank skull when you reincarnate which can be used to re-attune to the PoP. Acaos

I have a character that has attuned to each "first portal" and 2 "second portals" in The Abyss. I am thinking about reincarnating him soon. I accept that I will lose attunement to the PoP. Will I also lose attunement to the first and second portals?

basically it is identical to losing the skull for hell runs except you physically keep the skull.

When reincarnating an Arcane Archer who has all the DB and Phleg bows within his Legendary Archer Bow, do I have to separate them all from the Legendary Archer Bow and bank them all?

I have just reincarnated a demi 3 toon to a staffmaster. I took 16 intelligence and, naturally, memorised up to level 6 spells. When I went to recoup my demi-bonuses the Thingy informed me "You must have enough intelligence to cast all your spells to claim demigod bonuses."
Does this mean that it is anticipating (correctly) that I will want to cast up to level 8 spells (as a level 15 wizard can do) and MUST therefore have 18 intelligence?
Is there an explanation of this on the forum since I couldn't find it under reincarnation FAQs and the build examples seem to cater for demi-god bonuses augmenting spell level abilities.

This is an anti exploit to prevent possible cheesy reincarnations. You wouldn't go from level 1 through two visits to Asmodeus with a gimped character just for a builds sake. Therefore you cannot reincarnate using 2x demi bonuses to make such a build work out. So you are correct in your assumption here.

Question regarding reincarnation: if I put a build into the reinc slot but do not make a new character, do I still have 1 week to complete the process, or is the 1 week from when the new character takes the slot?

i made that mistake once and lost my first even immo that way. the week goes in when you start the reincarnation process, ie right after giving a character up to the light.

Is there a readily available table for the xp cost/loss of reincarnating by xp or level threshold?

NVM: This is a table of the XP loss. Funky confirmed that it was a linear scale from 12.5 to 125, decreasing from 20% to 1%. XP_m XP_loss% XP_loss_m 12.5 20.0% 2.5 25 17.9% 4.5 37.5 15.8% 5.9 50 13.7% 6.8 62.5 11.6% 7.2 75 9.4% 7.1 87.5 7.3% 6.4 100 5.2% 5.2 112 3.1% 3.5 125 1.0% 1.3 This is the equation to tell you exactly how much XP you will lose at a given XP level. XP loss = XP*(21+(10/9)-XP/12.5*(1+10/9))/100

This might be a good table to put in the OP.

When I'm reincernating my Paragon Staffmaster that have +2 INT from Paragon levels, is this considered in the described anti-exploit or not?

That follows the same rules as Demi Bonuses, I believe. If you're using paragon levels for +2 int to cast spells, it's the same as using a Demi bonus that way.
