Building on ubernewb's suggestion and ideas like the Accursed Pariah's healing, what about items that are better than UR quality but have serious negative side effects in the form of preventing usual practices to balance them out? Thinking something like:
Portrait of the Mason[Belt] Con +12 Disc + 15 Universal Saves +5 Immunity to Implosion Immunity to Fear Immunity to Ability Drain Immunity to Mass Camouflage Immunity to Lesser Restoration/Restoration/Greater Restoration Immunity to Negative Energy Protection Immunity to Shadow Shield When you see it, you'll **** bricks.
Similar to, or expanding on, the idea of upgrading the class secrets, how about cursed items in the Final Fantasy 3 sense where the item is crap but if you wear it for x number of battles or do something special with it, the curse is broken and it becomes uber (for example, instead of just clicking on a pedestal, if you are actually wearing the item when a higher level boss like Asmodeus is defeated, his curse on it is broken and it becomes hells or bur quality)?

Boots with multiple instances of +AC (over +20), to counteract hells/debuff penalties. Yes it works, I tested it on dev.

Would it be possible to restrict items by subrace? If so, perhaps some really uber gear that only non-BUR/UR races can equip?

Ur quality rings with immunity aginst the new damage types, either in a combination with elemental like ur rings or just immunity to the new damage types. (Ecto, internal and so on)
More items with immunity to fester.
All in all, more items with immunity against new damage types.

Armour of the Conquered
Some people just don't mix classes sensibly, and from the oddball collection of stats on this piece of armour, it is quite easy to tell that whoever it used to own it should have consulted the builds forum.
Light Armour (+17 would be nice but as I take it the loot is meant to be less than BUR quality +16 would probably be more in line)
+ 2 on Dexterity Checks 15% Physical Immunities 10% Magic Immunity + 20 Taunt + 20 Listen + 10 Concentration + 10 Discipline + 10 Tumble 3 x Level 8 Sorcerer Spell Slots 3 x Level 9 Sorcerer Spell Slots
The magic immunity would make this light armour usable by almost any tank wearing light armour. I chose the taunt and listen bonuses as very few items seem to give any boost to them at all, and it would be nice to allow a bit more flexibility in gear whilst still boosting these skills. Discipline and tumble seem to be fairly standard on new armors, so added them in. The sorc spells are to give some of the sorc quasis a light armour with spell slots to balance the medium and heavy options already present. The + 2 on dex checks also appeals to these casters, or even druids looking to bump their checks up. The nice immunities are balanced by the lack of bonuses to stats or immunities (death magic etc), which would put a bit of a squeeze on other gear slots when using this armour, which would hopefully keep it in balance. Tried to make it reasonable, but it could be way overpowered, my first go at item creation.

How about Shifter/Druid item's that improve the DC of some of the shifted forms DC's while shifted, or grant new ones. Cant wait for the shifter changes

Lesser Belt of Eternal Preservation.
Crit immune half the duration of the BUR belt. Change the greater restoration to 5x regular restoration.

Bracers of Greater Descent GMW +14 Keen 30th lvl Darkfire
Amulet of Greater Descent Any 4 buffs cast on character while item is equipped will be stored for a single use at a later time of the players choosing. Un-equipping the item deletes the buffs stored inside.

Shield of Revenge
base item: large shield
AC bonus +14 (to equal with URs) +20 to Parry +10 to Discipline 5% piercing + slashing + bludgeoning 10/- piercing + slashing + bludgeoning
- Death Armor 3/day at clvl 41 - when the wearer of this shield is struck by melee or ranged attack, there is 10% chance that the attacker will receive 10d10 of single random elemental or physical damage - you must not be Good to use this shield - you must have at least 18 base Constitution to withstand immense powers of this shield

I've always been a big fan of the spell 'Wish' and the 'deck of many things' in regular DnD... as I've found out, HG already has a deck of many things but its kinda a wimpy one... so maybe a non-memorizable wish scroll or an improved deck of many things (with some very nice bonuses and some very harsh irreversible things)

- drinking this potions will make you completely immune to all spells of selected school, both harmful and beneficial, for a short period of time (5 or 10 rounds, up to dev) - these potions are highly addictive and you may not drink any of them more often than once per rest

Back on topic... Robes of the Forsaken (Similar to the Echoes of Creation and the Uro light armor) They let you cast EV as a PM.

Not sure if this is possible, but boots which grant extra blackguard spells? Something Like
Boots of The Unholy Some people choose to walk the path of darkness. Rumour has it that it is a long walk, so a good pair of boots, such as these, would be a wise choice
