I owe inspiration for this build to a thread in the Drunken Monk tavern in which someone said something like, "shadowdancer and dwarven defender offer nice bonuses to monks."
Dwarf - Derrzagon (no access to secret subs yet)
DEX +2, INT +2, WIS -2, CHR -2, SR18, free feats: Blind Fight & Keen Sense, resist acid 5/- resist fire 5/-
Abilities: initial / subrace / lvl 40
STR: 10 / 10 / 10
DEX: 18 / 20 / 32
CON: 16 / 16 / 16
INT: 8 / 10 / 10
WIS: 14 / 12 / 12
CHA: 6 / 4 / 4
I had two extra stat points at creation that could be used to up str or int by 2, or wis or con by 1. I chose str, as I had no idea what to do with extra skillpoints.
This is the single-kama version. I used the feat slots that would otherwise go towards ambi / twf / itwf for the pre-epic save feats. There were four slots to play around with, so if you did go the two-kama route, you still have one extra feat to use for a pre-epic save or something else.
I figured, with haste and flurry of blows, it already has 8 attacks per round at -2 to all. The two-weapon route knocks another 2 off of ab for another 2 attacks... or it could have 6 or 7 more points in saves. I personally chose survivability.
All stat points into Dex.

The DwD levels give you Defensive Awareness, aka Uncanny Dodge; also you cannot be flanked, which if I'm reading it right, means you are immune to sneak attacks. Not sure how common sneak-attacking mobs are at higher levels, though. If being unflankable is not important, you could swap the 5th DwD level for a 6th level in Fighter, which would get you another bonus feat.
I originally was playing around with Shadowdancer as a splash class, but then realized that the only things it gets you are HiPS and Epic Dodge - at the cost of 10-20 hit points, BAB, another 8 skill points (move silently), and another feat (mobility; more if you replace fighter levels with it). I know ED is nothing to sneeze at, but I decided on the alternative for no specific reason. However, if you have a non-dwarf-based secret subrace, replacing the DwD with Shadowdancer could be beneficial - but then you potentially run into xp penalties with the fighter levels.
