Got the limbrot and my head and neck slots fell off in the fight
thereby not having confusion immunity and immediately getting confused. I got added back to the party literally just after the boss got killed. I got the acc for killing Demogorgon (in '!list acc') but no wand for the kill when checking the skull, just attuned to 2nd portal in GM only. I was present for the entire run and didnt miss tagging the 1st portal or any of the minibosses. DM chainlink was present and can verify.
This was this characters first ever abyss run so definitely no question of having it before, and it was not a prince fight either.
This has happened several times recently in my experience so being confused/out of party is not granting the wand as was supposed to be.
