This has been a topic of great discussion for the McFobs on teamspeak, and I think that Ithgar issued a good post, based on his experiences at other servers, at mcfob.com that could serve as a starting point for balanced pvp on this server.
Any thoughts about a system like this?

My ideas on PvP and GvG
Type 1 FvF (Faction v Faction)
FvF (Red v Blue) last man standing. Players battle till only 1 char alive on 1 side. ( open to anyone who sets faction (Red v Blue) reguardless of guild))
This can be set by an NPC at the arena gate.
FvF (Red v Blue) CFT (Capture The Flag). Players take a faction and try to both defend there flag and capture the other teams and return to you flag with it to make the capture).
FvF (Red v Blue) CTK (Capture the Keep). One faction takes positioin in keep and fends of agressors to a multi part break in pushing the defenders further into the keep till aggressors can hit a special lever to indicate full breach.

Type 2 PvP (player v Player)
PvP (Hide n Seek) Each Player recieves token and enters arena made of large blocking structures scattered around and seek eachother. First one to kill the other gets that players token and automaticly ported out and wins the match. Matches can consist of single kill or best of 3.
PvP (Gladiator) same as (HnS) but pit battle style. can also be single or best of 3 kill.

Type 3 (Guild v Guild)
GvG (BattleField)
Guild members enter a battle field style arena first guild to kill all members of other guild wins.
When you are killed your arena token is removed and the dead get auto ported out of area.
GvG CFT Same as Type 1 CFT but members of opposing guilds
GvG PvP Same as Type 2 PvP but members opposing guilds
GvG DTC (Defend The Castle) Same as Type 1 but members of opposing guilds

PvP status and ranking system can include.
Top gladiators get special personal room at gladiator hall.
Top gladiators get gold and items per weekly rank status out of pre slected list from hall merchant.
Top gladiators get access to merchants that sell items (taged as non drop or trans) that are same as rare items found in game.
GvG status and ranking system can include.
Top Guilds get upgrades to guild.
Top Guilds get features added per there ranking
Examples.
Ring recharger for No.1 guild that auto moves to head guild on rank list
Mass seller in guild.
Guild members having access to special merchants much like gladiators.
Any thoughts about a system like this?
