Hi,
A long time ago, I put together a Shapechange chart, I'm posting it here as it may be of use to others. After seeing it, I'm sure you'll note where my inspiration to create the subrace chart came from .
I hope the chart format below comes out ok on here, I used the "pre" tags. To print it save it to your drive as a txt file and drag it into IE. Make sure all 4 margins are set to .25" and its set to landscape mode and it should print nicely on the front/back of 1 sheet of paper (even if the formatting doesn't look right on the BB).
I may post updates to this thread if new/corrected information comes to light. I hope others find this of use too.

This data was compiled from a variety of sources, with the main source being the Shifter FAQ. I can't vouch for its
complete accuracy, but it has proven to be a very useful source of information for my Druid and Shifter builds.
W/I/A: They denote whether (W)eapon, (I)tems, and/or (A)rmor merge into the form. In the case of W it also denotes
wether or not the form uses a Weapon or Unarmed combat. If it uses Unarmed, the weapon properties don't merge.
Weapons: If a weapon merges, all properties (damage bonus, keen, stat/ability bonuses, bonus Feats, etc.) merge, but
not the base weapon damage (i.e., Longsword's 1d8) or magical bonus To Hit. Note that the magical damage bonus does
merge, so a +3 weapon merged into a minotaur's battleaxe will become a +1 battleaxe with +3 damage. Note also only a
handful of Shapes merge weapons and the merged properties don't always show in the Character screen.
Armor: Includes Shields/Helmets. All properties merge except the item's base Armor Class. Bonuses to AC, skill
modifiers, and bonus Feats are melded.
Items: Include Cloaks, Belts, Rings, Amulets, Boots. All properties from these things merge if the form merges items.
Gloves/Gauntlets/Bracers/Offhand Weapons: These items never merge in any form.
