The elusive 6th - monk attack

can anyone advise me of the correct feat selection,skill point distribution or ability stat that is required to achieve the elusive 6th attack per round in a monk build. I can only seem to get as far as 5 attacks. I have managed to do it once in an editor, alas, I was not making a note of my selections and have not been able to recreate a similar build since.
All advice would be greatly appreciated

Its not that elusive, just make sure your BAB is +16 or bettre by L20.
