I waited until over 20 to take cot lvls to take the extra epic feats cot offered, and if you take any monk levels dont until over 20 (at lvl 37 if only 1 monk)

Yes, it would give you +2 to your reflex save.

Since Paladin and CoT are both full AB progressions, you can take 19 Pal/1 CoT for the first 20 and still get your 20 BAB.

my saves currently are well into the 60's or 70's so +2 doesnt matter much

CoT saves are included in the cap so i wonldnt bother making build choices based on CoT saves. You do get 1 more reflex from the CoT primary save though.

You do get 1 more reflex from the CoT primary save though. All together 19 Pally, 1 CoT versus 20 Pally for your first 20 levels gives you +2 reflex save and +1 Fort save. I would agree if there are cons to doing so, in that case I'd probably let the cons decide. However, in this case its all good and no bad, so do it. Its a few save points and like AB, the top few are the best (needing a 18 is 3x better than needing a 20 . Its also less items/such you need to get to be at the max. Dave.

I usually start a build with what outside classes I may want involved. Here are some good options: Monk - Evasion, Cleave, Tumble Rogue - Evasion, Tumble, UMD Champion of Torm - feats, including Great Smite Harper Scout - Tumble, Great Charisma Shadowdancer - Evasion, Tumble Weapon Master - Better crit damage
If you want WM, take 13 Int and Expertise at creation time, so the race change takes you to an even 12. (Unless you see more than 4 skills you want to max out.) WM will also require 15 (taken down to 13) Dex. If you don't go with WM, determine how many skills you need to max out, and use (2+1+Int modifier) to see what Int you need (probably 10 or 12).
At creation time, you get 30 points for your base stats. The first 6 increases are 1 point each; the next 2 are 2 each; the last 2 are 3 each. Here we see that WM will come at no small expense (8 points to add 7 Dex and 5 points for 5 Int). Your Str, Con, and Cha are good priorities.
I'll leave it at that for now. Creating a build is part of the fun, so I don't want to push too many of my own ideas on to you.

1-19 pally 20 cot, 21-27pally, 28-36 cot 37-38 monk 39-40 pally is a pretty standard build on this server, All your saves will be 65-70's at 60 at least 26 chrisma (if you want great smite x), rest into str

Quick question: Why do you take the two levels of Monk? Aren't their abilities nullified in armor? -- is it just for Tumble?

must have been a typo, probably best to take monk at level 40 now to max discipline and take 40 in tumble. So 1 monk 10 cot 29 palidin.

The reason for taking the 1 monk level is for +8 to ac from tumble and evasion. With the reflex saves that a paladin has they will only fail on a 1.
